Partners and plans
We wrote the standard. Evaluate us against it.
For assisted living, skilled nursing, health plans, hospitals and physicians: home health, palliative care and hospice from one organization, across four Southern California counties.

Who we partner with.
Assisted living
Your residents get to stay home. Home is your building.
Every move-out is a family you lose and a room you refill. We bring the whole continuum into your community, with one clinical team in your building every week: home health after a hospital stay, palliative care for residents declining between levels of care, and hospice that lets a family keep their mom in the room she knows. It costs your community nothing; Medicare, Medi-Cal and health plans cover the care.
A Care Continuum Manager in your building
A licensed vocational nurse assigned to your community and on site twice a week: rounding with your charge nurse, screening for decline, keeping resident binders survey-ready, talking with families early and working with each resident’s physician. One point of contact for your team, and for ours.
- AssessWeekly rounds with your nursing staff
- IdentifyDecline flagged weeks ahead
- EngageFamilies and physicians, while the resident can still speak for themselves
- InterveneHome health or hospice while the resident is comfortable
- PreventThe 911 call that never happens
What the rules and numbers say
- § 87609 California lets your community keep a resident whose condition needs nursing care when a licensed home health agency provides it, under a written agreement on the services, how often and for how long. California Title 22 (opens in a new tab)
- § 87632 To keep a resident on hospice, a community needs a hospice care waiver from the state and a care plan agreement with the hospice for that resident. California Title 22 (opens in a new tab)
- 4 in 10 assisted living residents live with Alzheimer’s disease or another dementia. NCAL (opens in a new tab)
What we bring
- A Care Continuum Manager on site twice a week, your single point of contact
- Home health, palliative care and hospice from one team
- Binder audits and Title 22 tracking on every visit, at no cost to your community
- The written agreements your license and your hospice waiver call for, worked out with your staff
- A person on the line 24/7 when your staff needs clinical backup
How it starts
- We come to you for an orientation visit with your administrator and director of nursing.
- We sign the partnership agreement and introduce your Care Continuum Manager to your team.
- The first binder audit and eligibility screen happen within 7 days, then visits twice a week.
Skilled nursing
Discharges that stay discharged.
A bounce-back is a penalty for you and a setback for the patient. We take your discharges the same day when you need it, with home health that reports back to your team, palliative care for the patients who should never have been readmission candidates, and hospice when the goals of care change. Your discharge planners get one number for all three.
What the rules and numbers say
- 2% of every skilled nursing facility’s Medicare Part A payments is withheld for value-based purchasing, and about 60% of it is paid back as incentives for how the facility performs. CMS (opens in a new tab)
- FY 2027 Discharge to community and long-stay hospitalizations join the program’s measures, with readmissions during the stay that could have been prevented added in fiscal year 2028. CMS (opens in a new tab)
What we bring
- Same-day admission when you need it
- One intake line for home health, palliative care and hospice
- Home health that reports back to your team
- Hospice when the goals of care change, without a new company to call
How it starts
- Your discharge planner calls or sends the secure form, any hour.
- Intake confirms the order and coverage the same day.
- A nurse visits within 48 hours of the patient getting home, and we keep your team posted.
Health plans
Right care, right place, lower total cost.
Your members with serious illness drive your hardest spend: avoidable ER visits, late hospice referrals and care split across agencies. We run home health, palliative care and hospice as one partner, with one record and a culture your quality team can hold to its own bar. Fewer sites of care to manage, earlier transitions, and members cared for where they want to be.
What the rules and numbers say
- SB 1004 Medi-Cal plans cover palliative care for members who qualify, alongside their treatment, and since January 2024 D-SNPs have to run palliative care programs for dual members. DHCS (opens in a new tab)
- 19 days was the median hospice stay for Medicare decedents in 2024, the year a record 52.9% used hospice. Half of them had less than three weeks. MedPAC (opens in a new tab)
- 33% lower costs in a randomized trial of in-home palliative care at Kaiser Permanente, against usual care. Coalition for Compassionate Care of California (opens in a new tab)
What we bring
- Home health, palliative care and hospice under one agreement
- Palliative care with the services SB 1004 requires, from advance care planning to pain and symptom management
- Medi-Cal and Medicare members
- 4 counties: Los Angeles, Orange, Riverside, San Bernardino
How it starts
- Leadership walks your team through coverage, the service area and the agreement.
- Your care managers get one line and one secure form for every member.
- Members move from home health to palliative care to hospice without changing companies.
Hospitals and physicians
Home, and staying home.
A readmission costs your hospital and costs your patient. We take referrals any hour, see patients within 48 hours of getting home, and stay with them across home health, palliative care and hospice, so the next thing you hear from us is an update. For physicians, the paperwork comes ready for what Medicare asks, and you hear from us from the first visit on.
What the rules and numbers say
- 3% of Medicare base payments can be cut for too many 30-day readmissions after heart failure, heart attack, pneumonia, COPD, bypass surgery, or hip or knee replacement. CMS (opens in a new tab)
- 90 + 30 Home health needs a face-to-face visit in the 90 days before care starts or the 30 days after, and a physician, nurse practitioner, clinical nurse specialist or physician assistant can certify it. 42 CFR 424.22 (opens in a new tab)
- 6 months Hospice starts with a physician certifying a life expectancy of six months or less if the illness runs its normal course, with a short narrative in the physician’s own words. 42 CFR 418.22 (opens in a new tab)
What we bring
- Referrals any hour by secure form, phone or fax
- Intake answers the same day, and a nurse visits within 48 hours
- Home health, palliative care and hospice, so the patient never changes companies
- Updates to the physician from the first visit on
How it starts
- Your case manager or office sends the referral by secure form, phone or fax.
- Intake confirms the order, the face-to-face note and coverage the same day.
- A nurse visits within 48 hours, and the physician hears from us from there.
Accredited and accountable
ACHC accredited and Medicare-certified.
And a member of the national and California associations for home health, hospice and palliative care.

Why it lasts
Every operator claims quality. Ask what keeps it.
Our answer is mechanical, and it starts with culture, the most important clinical variable we manage. A measured culture keeps clinicians. Clinicians who stay keep one record across three lines of care. That record earns the trust of families and the people who refer them, trust builds density across four counties, and density funds the culture.
The wheel has been turning since 1967. A lot of this industry is built to be sold in three to five years. We’re building for a lot longer than that.
Why we believe itTurning since1967
- A measured culture Checked every year by an anonymous survey
- Clinicians who stay The people families meet keep coming
- One record Across all three lines of care
- Trust From families and the people who refer them
- Density Across four counties, which funds the culture

Questions
Questions partners ask
Which plans and populations do you serve?
Medicare, Medi-Cal, CalOptima and other managed care plans. If you’re weighing a new agreement, leadership can walk you through coverage and the service area in one conversation.
Where do you provide care?
In Los Angeles, Orange, Riverside and San Bernardino counties, in private homes, assisted living and board and care homes.
Can you take a discharge the same day?
When you need it, yes. Call (562) 600-1000 any hour, or use the secure form on the Refer page.
Can a nurse practitioner order home health?
Yes. Medicare lets a nurse practitioner, clinical nurse specialist or physician assistant certify home health. The face-to-face visit it requires can happen in the 90 days before care starts or the 30 days after, by telehealth too (42 CFR 424.22 (opens in a new tab)).
Can a resident on hospice stay in our assisted living community?
Yes, when your community has a hospice care waiver from the state and a care plan agreement with the hospice for that resident (Title 22, § 87632 (opens in a new tab)). We work through the agreement with your staff.
Is the partnership inquiry for patient referrals?
No. It goes to HealthView leadership and holds no patient information. Referrals go through the secure form on the Refer page, the phone or the fax.
