Questions
Questions referral partners ask
Can I refer at night or on a weekend?
Yes. The referral line, (562) 600-1000, is answered by a person 24/7, every day of the year. Hospice and palliative care share their own line, (626) 505-1000, answered the same way. The office keeps weekday hours, Monday to Friday, 8:30 AM to 5:00 PM, but referrals never wait for Monday.
Which insurance do you take?
Medicare, Medi-Cal, CalOptima and other managed care plans. Not sure about a plan? Intake checks coverage in one call.
Where do you provide care?
In Los Angeles, Orange, Riverside and San Bernardino counties, wherever the patient lives, including assisted living and board and care homes.
Do I need a doctor’s order first?
Home health and hospice both start with a doctor’s order. If you don’t have one yet, send the referral anyway and we’ll work with the doctor’s office to get it.
What if they need a different kind of care than I asked for?
Send it anyway. HealthView runs home health, palliative care and hospice, so intake points it to the right team, and the patient keeps the same organization if their needs change later.
Can families refer?
Yes. Families can call any hour or use the family form on this page, and we help with the doctor’s order.
Is it safe to send patient details here?
Use the secure form, the phone or the fax. The form is HIPAA compliant and encrypted, and it goes to intake through Jotform, not through this website. The Care Navigator chat is for general questions, so it never asks for names and keeps nothing.
